Privacy Policy

At DJN Physios we are committed to protect and respect your privacy. This policy sets out how DJN Physios handle the personal data of our patients from the types of information we collect to how the information is stored.

How do we collect your information?

We collect your information when you book in for an appointment whether this be through online bookings on our website, over the phone, face to face or via email. Further information is collected on arrival at the clinic through the use of a new patient form.

What type of information is collected?

The personal information we collect includes: your name, address, contact number, occupation, health details-allergies etc. We only collect information we require for medical diagnosis and treatment and that we deem relevant for completion of treatment.

In terms of payment if you are self-paying your card details are NOT stored. If you are using insurance, we require all personal and insurance details in order to process invoices and complete treatment.

How Is your information used?

  • To gain further information about you to aid with diagnosis and treatment
  • To notify/remind you about appointments or changes to your appointments
  • To gather and analyse information so we can improve our service
  • To chase outstanding payments
  • To provide customer support
  • To provide you with general information about our services
  • For onward referrals to consultants, specialists or any third party

Please note without this information we will not be able to contact you if there is a change to your appointment or if your treating practitioner is unavailable. Additionally, if you choose not to receive a reminder and miss your appointment there will be a late cancellation/DNA charge.

Who has access to your information?

Your data is only shared with the reception team and treating physio when required. We will NOT share your information to third parties for marketing purposes, if data is shared for an onward referral we will contact you to gain further written consent before passing anything on. All data provided is strictly confidential and NOT shared with anyone, it is stored within a password protected database (Cliniko) which can only be accessed by each individual staff member.

How long do we keep your data for?

Whilst you are using our clinic we are legally required to store and use your personal data to enable us to make an accurate diagnosis and to create an appropriate treatment plan. All records are placed on Cliniko and you have the right to request a copy at any given time.  Once you have been discharged, we will be required to retain your personal information indefinitely to comply with the medical records act.

What are your rights?

As we process your personal data, you have certain rights. These are a right of access, a right of rectification, a right of erasure and a right to restrict processing.

You may request a copy of the information that we hold about you at any time. If you would like a copy of your medical notes or any of the information we hold on you, then this must be done in writing by emailing Please provide us with the following information: your name, address, contact number and details of the information you require.

We want to make sure that your personal details are up to date. If any of your personal data changes or you believe it is inaccurate or incomplete, please contact us and your data will be updated promptly.

You also have the right to make a complaint if you feel we are not handling your data correctly or object to the Information Commissioners Office at or by phone on 0303 123 1113

What happens in the event of a data breach?

Should your personal data be lost, stolen or breached we will contact you without delay. We will provide you with the contact details of the Data protection officer who is dealing with the breach, explain to you the nature of the breach and the steps we are taking to deal with it.

We keep our privacy policy under regular review and will bi-annually update this policy.
